Such a button requires a macro. Is your recipient going to allow it to run?

If so, instead of a button on the form, you may want to simply add a QAT icon for your macro.

Thanks for your help, Charles. Ultimately, I want to create a pdf from this form. Would the submit Mailto: still function?

That is actually a hyperlink.

It may or may not be preserved in the pdf; this would depend on how you save the pdf.
If you have a pdf editor, which you would really nead to create a fillable pdf form, you can do this in that editor. Word would just give you the framework or backdrop.

I'm using Acrobat Pro to create the PDF. Creating the button in Acrobat removes all of the form's functionality. I was hoping if I could accomplish this in Word, it would remain functional in my PDF. Guess not. I appreciate your help!

Regarding Mailto:, it's used in the function of the Submit button I created.

Microsoft Word form functionality won't work when you save to a PDF. Use Word to create the static content and then when you save to Acrobat, add the form functionality in Acrobat Pro.

Yes, that has been my experience, and what brought me to this forum. I can't figure out what I have to do to create the button in my .doc. When an applicant has completed filling out the form, I want them to be able to click on a Submit button that will email it back to me. ty

You can use Word to add the 'artwork' of a submit button but I would just leave a space on the form and do it all in Acrobat.
There are videos on youtube showing you what you need such as this one https://www.youtube.com/watch?v=1iczBlPYREE
