On the References Ribbon Tab there is a Footnotes Section. Click on the dialog launcher icon in the bottom right corner of that section of the ribbon - it opens the Footnote and Endnote dialog where you can specify how many columns you want for the Footnote Layout.

There is also a different way to reach the Footnote and Endnote dialog box: You can right-click the footnote text, in the footnote area at the bottom of the page, and choose Note Options from the context menu.
Keep in mind that if you are saving to the older *.doc format, multi-column footnotes are only supported if the whole document is formatted with multiple columns.

Did you try the Footnote and Endnote dialog box? As long as you save the document in the *.docx format, you can change the number of columns.

Thanks for the sample. I had to "upgrade" the document to the most recent version of the *.docx format for the footnotes to display properly in two columns. I am attaching the document.

I re-saved the document so that it is no longer in compatibility mode. After that, I could use the Footnote and Endnote dialog box and specify a two-column layout for the footnotes. I don't remember exactly which version introduced the possibility to specify multi-column footnotes (even if the main body of the document is a single column). Is it possible that your document was intentionally saved so that it should be compatible with older versions of the Word file format?
