Text boxes for individual characters
I'm making a form that people have to fill out. How do I insert a grid of boxes for individual letters and numbers like in the attached example.

That can be done in Acrobat but not in Word as far as I know.
If I HAD to do it in Word, I would make a table graphic to sit in the background and put a Content Control in the foreground with a monospaced font and a whole lot of extra font spacing.

If it is a handwritten form, use a Table. Otherwise Andrew has answered.

You might also want to adjust the layout so it's clear where family names and given names are to appear. Otherwise, you're liable to have some people putting family names first and given names second, but other putting given names first and family names second. Likewise with date of birth - which order should the day, month, and year be listed.
