So the "pleadingpaper-blank-menu" document achieved the lines and numbers by placing the objects in the header. The vertical lines are just line objects. The Numbers are a Text Box without borders.
I am using O365 and would number the Lines from the Layout Ribbon. There is an Option under the group "Page Setup" called Line Numbers. Reviewing my copy of the V6 Word manual, File, Page Setup, Select the Layout Tab and choose the Line Numbers button. Line Numbers will be added to the current Section. |

For more on this, I would recommend posting a separate question. Although very much related, you are NOT using Word 97 and that can make a big difference.
