Help with formatting form to fill out on computer
I cannot work out how the attached form is formatted. The checkboxes are impossible to align and if I were to delete the starting sentence, all but one of the checkboxes disappears! What's going on, please?

The checkboxes and grey boxes are simply floating graphics sitting on the page. They are not associated with the table cells. They are nearly all anchored to the Question paragraph (Select that paragraph and note which ones get selected).
The question is do you expect the users to complete the form electronically or do you want them to print it out and write on the piece of paper. This layout is purely for printed form completion. If you wanted to complete the form electronically, you should delete all those shapes and replace them with Content Controls.

I have tried selecting the question para but I don't see any floating graphics getting selected.
And what are Content Controls? I should say that my Word version is 2003. Does this have content controls? |

Word 2003 does not have or recognize Content Controls.
Here is a screenshot where I selected and moved the table. It shows most of the graphics did not move with the table. The one for Secure Loans is actually shading in the table cell rather than a separate graphic. You could use just a Table to handle shading. You could use Form checkbox fields instead of the box symbol, but if you want them checked or unchecked on the computer more is required. OK Thanks Charles. Problem is, this form was created by someone else and I'm trying to complete it, but it's impossible to do it on screen. I was hoping to modify it so that I could, but I think it's better to print it and do it longhand! Thanks anyway.
