Printing 4 pages on a4 sheet

Dear all

I have 40 pages (4" x 5") in a file. I want to make a booklet of these pages.

Please let me know to to organise printing of these pages 5 on 5 A-4 sheets.
4 on side side. These sheets may be cut in such a way that pages are in sequence.
