Default Numbered lists out of sequence

I inherited an 860+ page document written in Word from another tekwriter. It is filled with numbered lists and sub-lists (1, 1.1, 1.2, 1.2.1, 1.2.2, 1.3, etc.) I recently had to update the doc with about 25 pages worth of new content and after generating the PDF and delivering it, someone pointed out that the numbered lists throughout the guide were screwed up.

For example:

This occurs in sections I didn't even touch. I went through the whole guide and reapplied paragraph tags where necessary to get the numbers back in line. A few days later I had to do some other corrections and regenerated the PDF and guess what? The numbered lists are screwed up again, in different places from the last time.

Why does this happen? Is there any way to perform a global fix on numbered lists so I don't have to spend countless hours paging through the whole doc manually fixing them?
