Hi
I have a form I just need it reproduced as it is as the original got wet and doesn't look so great
I can email it to anyone willing to help

hehe yeah that is an obvious answer.

If that's not possible then you'll have to do it yourself Tamoshanter, by building a form manually.

I can't imagine many people would be interested in doing such a job for you, especially urgently, and especially free.
If you are willing to pay perhaps try Airtasker etc....
