Default Forms: combination of a list and a text field to be filled in

Hi there!
Is there a way to implement in a word 2007 document a combination of list/combo with a text field?
The case:
The user is given a choice with only two options (and that's how the list/combo should look like):
1. Amount sold [here goes a text field to type in a number - the amount]
2. Percent sold [here goes a text field to type in a number representing %]

Appreciate any help - even other ideas...

Cheers,

Bart
