Pop-up window to input data in diferent places of the documment

Hello, i have a document (screenshot is attached) is there a way to make each time I open the document (or when I press some combination) a windows would pop-up and ask to input info and put the info into different places (marked yellow in attachement)

First, you want to be using a [.dotm] template rather than a document. This is Word jargon but important. Templates in Microsoft Word When you double-click on a template you will create a new document. The template can have an AutoNew macro that activates a Userform (custom dialog box).
