Default Field code wierdness in document map

A document I have been using for over a year suddenly started showing the opposite field code setting in the document from that in the document map.

If a field in the document is part of a heading that appears in the document map, then if it is set to show field codes (Shift+F9), that entry in the document map will not show them. If I toggle the field code setting, it will be off in the document and on in the map.

If I select the entire document and repeatedly press Shift+F9, field codes will alternate between on in the document and off in the map and vice versa.

This happened while I was adding some new fields and assigning bookmarks to them. I was toggling field codes on and off until, at one point, I noticed that they were opposite in the map.

Any ideas what I did and how I can fix it?

I can't post the actual document because it contains confidential information, but if it is necessary, I might be able to edit it down to a few lines that exhibit the problem and post that.
