I am speaking from ignorance here. One thing I observe is that you are using (1) content controls (2) checkbox formfields (not content controls) and (3) ActiveX radio buttons. I would advise only using content controls in Word 2010 for your purposes. Content Control Custom Checkboxes

I do not know why some of your plain text controls have chevrons and others do not.

I note that, if the content controls lacking the chevrons are deleted and new ones are inserted in their place in an empty cell or an empty paragraph, the new ones have the chevrons. If you then add anything else to a cell or paragraph (e.g. a space before as the 'First Name' content control has) containing a content control having the chevrons, the chevrons disappear. Similarly, there are no chevrons if the content control is inserted into a cell/paragraph containing anything else. In either case, the chevrons do not re-appear if that other content is deleted. Other than that, the chevrons appear to be of no consequence.

Hey Charles and Paul,
I got it to work!!! (Happy Dance))
Your feedback pointed me in the right direction and it work...ALL OF IT!!!
I removed the Macros, deleted content controls that did not have chevrons and replace them without a space in front.
I also figured out why some of the boxes were typing in gray font and not black. Yuppie! Time to move on to the Men's Size card!
