Default Table of contents does not print as displayed

Hi,

I wonder if anyone can help, as this problem is driving me crazy and I can't seem to find a solution.

I'm using Word Office 365 and my document headings display correctly in both the main body of the document and within the automatically generated table of contents. However, when I print, one of the headings in the table of contents does not print correctly - it concertinas together with the page number. It always seems to happen with the Level 1 Heading in Chapter 3 of the TOC. If I change to a Level 2 heading, all displays correctly.
I've tried re-formating the Level 1 heading but that hasn't fixed the issue.

I've attached a sample document and PDF to show the problem

Thanks for any help you can give.
