Word allows multiple reviewers to comment on a document simultaneously. Then, the author can combine the comments to be addressed. This is extremely inefficient and disrespectful of the reviewers' time. I'm wondering if there is a better option. Is it, simply, setting a schedule so that reviewers are not duplicating each other's work, and always see comments and changes that have already been made? I'm looking for a technological return to the old (effective) method of placing a review sheet on top of a hardcopy document and passing the whole thing around for red ink markup!

If you work in an environment where your reviewers can and will accept a strict hierarchy and sequence, then that's fine.

Otherwise, an old-style review meeting lets all the reviewers become aware of each other's input. As the person producing the document, you can streamline the meeting by consolidating comments beforehand so you only spend time discussing differences of opinion.
