Default Format Object?

I commonly create an envelope for documents I create. Each time though, the same name and address shows on the left side middle of my envelope. Where is this coming from in the first place and how can prevent it from appearing.

Go to the Mailings tab, click on Envelopes, and check whether you've specified the address in the dialogue box there. If so you should be able to delete it.
