Default Creating a User Form for Word 2010

Everyone,

Can I get some assistance to create a User Form for Word 2010. I
am going to attach the form and the requirements for the form. Can
someone give me an example that I could play with, test, edit and then


save the form? Once I have an example of what I am trying to do, I
should be able to figure out the rest. Thanx in advance.
