Default Quick Parts Fields?

I wish to create a very basic form. All I want to do is to put an empty "field" where presently are some underscores or lines where the user is to fill in information.



This is accomplished almost perfectly by using Developer Design Mode and inserting a blank text field from "Legacy" Tools. But I don't want to be stuck in "Legacy", I want to learn what the heck I should be using in this generation.

After a lot of searching my "Insert" ribbon I found "Quick Parts", as all I am showing is a little icon squished between other icons (even though I am running a high resolution wide screen) and found "Fields". I started inserting the field labeled "autotext", not to be confused with the "Quick Parts" "AutoText" selection (...), but once one of those fields is inserted I can not place anything else on that line - it kicks everything down to the next line - so that can't be right.

I just have a few lines that I want to result in something like

Name:_____________ ID#:_____________ City:___________

but with fill-in fields instead of lines-

What should I use?

Once again I embarrass myself. I used those less than a month ago, I was at that site less than a month ago, but I couldn't remember either! Content Controls were not showing up as I was trying to edit a form created in an old Word release and so the doc kept opening in "compatibility" mode and wouldn't update when I saved it, so I copied and pasted the text onto a new doc blank and voilą! Content Controls are available!

Greatly appreciate the help. I would have just driven my blood pressure up. Will make a good bookmark this time.
