Default Formatting Forms!! Please help!

Hello. I have been given the task of creating new forms at work. I have created form templates before, but they want me to go off of a printed form that was not saved. This form kind of looks like this

Employee's Name Date Started

Every time I try to format it so its centered if you go to add information into the text section of the forms it shifts everything over. Is there a way to format the document so the text entry window above Date Started does not move when you enter Employee's Name? Am I just making things more complicated then they really have to be? I did look at some videos but they are only for single line fields, not multiple.

Boy I hope this makes sense! I need this done before 5pm today!!

I really hope some one out there can help me

PS... I cant seem to format the lines and the text in here to have spaces for an accurate picture of what I am looking for, but I hope you get it... Thank goodness its Friday!!

Hi Nikki,

Use a table (make the lines invisible) as framework for your form.

For your example, use two rows; the first for entering the name and date, and the second for your "Employee Name" and "Date Started" labels.
