Is there a blank CD inlay jewel case template

I'm trying to use Word but there is no blank template. All have designs.
Avery does have some on their website but they don't seem to operate correctly.
Trying to add text to a photo then rotating the text and can't find an editor that will do that.

Looked like it might work but doesn't. All I get is a blank office 8x11 blank document page
No format of the labels.
Guess I'm out of luck

The CD Stomper label templates for Word (in DOC format), linked by JimP, work just fine in all recent Word versions. They contain a table which has the required jewel case layout, preformatted for use. Click in the centre of the page and then ensure view gridlines is selected from the Table Tools Layout tab,
