Default How do I create a field entry that replaces a text?

I'm trying to create a form where people can insert their information.

For example,

[Name]
[Telephone Number]
[Address]

When someone clicks on these texts inside the brackets, the whole text is highlighted, and can be replaced with his actual name, number, etc.

How do I create these insert fields?
