Default Autofilling a box

Hi people,


I have been rejecting ALOT of documentation because operators filling boxes from a document Wrong, i wanted to know if there is a way to selec a box or
put a "X" in a box depending of a certain score number from other part of the document.


As you can see in the next image:



Operators must select a number between 0 to 3 and then depending from that number sele a risk score and mark with an "X" the check box,the boxes are merely a table so i dont really know if there is a command to do this automatically that would be awesome.



A im open to any tutorial, link or explanation, thanks in advance and have a nice day c:
