Default List of Figures, list of charts error

Greetings to everyone,

I have used Microsoft office especially word consistently over the past decades but I recently get into a problem I can't seem to understand the root.

When I create a list of figure, chart or table automatically, I immediately get a big list in repetition. This could turn a 20 page document to over 300 pages.

The document itself became too big that it cant be sent via ordinary email at just 27 pages. I am not sure why this problem is occuring and I have not found anywhere online where this same problem has been reported.

I have added a screenshot of the problem to further show what the problem look like.

I would appreciate any has suggestions or hints why this is happening? Thanks
