OK, the problem I am seeing is that the command is dimmed out. That is, it is on the menu but you cannot select it.

Yes. However I have gone into the proofing section of the general options menu and found that the roaming custom dictionary was marked as the default. Also, for some reason, French and Spanish also seemed to be involved.
I have now changed the default dictionary to custom and also made sure that English UK is the language. The 'add to dictionary' option is now active.
