Fill MS-Word docs/forms with name address data from external source

In MS Word 2003 onwards, what is the easiest way one can one pre-fill MS-Word forms with externally supplied data?
For instance, there are 7 forms published by a small claims court. Is it possible to fill the documents with entries supplied from some other source (example a text file) i.e. address in the address field, phone number in phone number field etc.
