Your footnotes appear to contain citations. How were they inserted? Did you use Word's citation & bibliography tools, or a third-party product (e.g. EndNote)? The format you're after appears to be the same as what Word uses for bibliography entries in the APA citation format.

Word does not come with a citation style that matches that format. You could, however, apply the APA referencing Style to the document, then copy & paste the references from the bibliography entries into your footnotes in place of the citation fields.

Do note that APA is an in-text referencing system only. Therefore, if you're using APA, you shouldn't have references in footnotes - they should be in the body of the document.

It appears whoever told you to do this may have confused the APA bibliography referencing with the Chicago 'Notes and Bibliography' citation system. The format you're now getting is apparently for APA in-text references. Note too that the Chicago Style specification supports two quite different formats: 'Author-Date'; and 'Notes and Bibliography', both of which you can see at: http://www.chicagomanualofstyle.org/...tionguide.html. Word doesn't implement either system correctly.
