Just upgraded from 2010 to 2013 and I have selected to show field codes gray "Always". Unfortunately when I do this, the field codes I have in my header show gray but do not show the text. What gives? The body of the document and footer are OK.

Note that text in the header/footer is also gray when you are not editing the header/footer. On your display this may be the same gray. It should be the same for footers as headers, though.

Oh bummer, thanks. My headers and footers are different for some reason!
