Default Placeholder Text showing in TOC

Using Word 2011 for Mac, saving in Word 97 2004 to get a doc vs dixc file format. Trying to comply with smash words style guide, therefore using custom header styles. When I try to link the chapter bookmarks to the line items in the TOC I'm getting placeholder text ({HYPERLINK.......}. Changed chapter heads to Heading 1 but same results. I am new to this but have followed several sets of instructions and get the same results.
