I have a word doc that I am using as an application. I want to be able to have users type in their name OVER a line. The line keeps wrapping onto the next line. See my example. THANKS

Instead of underline formatting, use table cells with fixed dimensions. Add or remove borders as required.

I created a simple example for the bottom part of your form. See the attached file.
Note that you shouldn't try to create a single table for all items. It's easier to use several small tables.
