Default Want a template to post online that people can fill out without editing the document

I have an order form for services performed by our company on our web site but it is at present a pdf and people need to print it, fill it out and scan it and then either email or fax it. I would prefer they be able to dowload a word doc that can be edited (in certain fields) so they can simply email that form to us immediately. I have tried inserting text boxes but I would like the underline at each line to remain.

Is there a solution to this or will we just have no underlining on our forms from now on??? Thanks for your help in advance :-)

Use the forms toolbar.
You can format the formfields with underlining.
Making Forms with Microsoft Word

You might prefer to use Adobe Acrobat and make pdf forms instead.
Word documents that are downloaded can give different results on different computers. What happens when I send my document to someone else? Will Word mess up my formatting?

If users don't have to sign the forms there and then (the sort of process where they place an order, you verify it and send an invoice or purchase order, they confirm or pay, you start delivering …) you could just use a web form.

Personally, it bugs me when I'm trying to do an online (therefore "quick") transaction and I have to go through the download, print, hand-fill, scan, send back routine.

Oh, and someone – Peachpit Press, I think it was – brought out a book some years ago with the title "Underlining is for typewriters" :-} Underlining on a web page is generally reserved for hyperlinks.
