Default Form Complete

I am currently putting together a form that users will have to complete and send to an authoriser for approval. The form will be a combination of Text Boxes and Radio Buttons.



All fields within the form are mandatory. Ideally, I would like to have an indicator at the top of the page to show when the form has been complete (i.e. all text boxes have entries and one of each radio button group has been chosen). Is this possible?

The form does have to be emailed to the same authoriser box, so the alternative would be to have a "email form to authoriser" button, which is only active when all fields have values.
