Hi,
I am working for a Dr.'s office today and we are stuck trying to modify our dictation templates. We have 4 templates we use for patients. They all have the Dr's info on the main header. We than add the patients name, date of birth and date of service under the main header, all subsequent pages must have the name, DOB and DOS at the top as well but not the main header with the Dr's info, (just the patient name, DOB, DOS and page #). We also need to have the page number on all subsequent pages of the document. We already have the templates set up. We just need the sub header info with page numbers programmed into the template. Can someone please explain to a Microsoft novice how to add this functionality to our templates. Or, better yet, I would be happy to pay someone (via Paypal?) to modify our templates with this functionality but we need this immediately. Can anyone assist on this please? ![]() Tony |

It's not clear from your post exactly where the patient data are added - in the header or in the document body. Assuming the latter, all you need do is:
1. Open the template for editing 2. Set the page layout options for a 'Different first page' 3. Apply a unique Style name (e.g. 'Patient') to the range containing the patient data 4. Temporarily create a 2nd page 5. Insert a STYLEREF field into the 2nd page header, referencing the 'Patient' Style, plus the Page # field 6. Delete the page break used to create the temporary second page. 7. Save the template

Pardon my lack of proper terminology but I generally just use "Word" to read documents or edit my resume. ;-) There is a header on the first page of each document that has the doctors name, qualifications and address. Just below this there is Patient Name: Date of birth: Date of service:. We just need the patient name, date of birth and the page number to appear on all subsequent pages at the top. We actually don't even need the words "name", or "date of birth". We would be happy if only the name of the patient, date of birth and page number appeared without Patient name, DOB etc. for example at the top we would love: Page 3 John Smith 3/14/1961 Thats all we need. If you have time, please explain it like I know nothing about Word.......... because I don't. ;-) Thank you so much. If you are willing to modify our 4 templates, I would be happy to pay you. Thanks! Although I could modify the template for you, you won't learn anything that way. Have you at least tried to do as I suggested? If you get stuck, post back with the details and we can take it from there.

I have to convert an entire practice to Electronic Medical Records and in house dictation and this is what they pay me to do, not figure out MSW. They asked me if i could easily figure this out for them or find someone who can do it for them. I try only to be on the clock for things I am an "expert" at. I'm sure there are plenty of competent people who'd do the work on a fee-for-service basis and I see you've had some offers in the cross-posted thread. I don't recognise the phone #, so all I can conclude you're not from the same part of the planet I'm from (there's no indication in your user profile, either). If the offers you've had aren't to your liking, I'd suggest digging out a phone book and checking out what services are available locally.

That is a NYC phone number. I spelled the last digit of each section as a bot avoiding habit. I have contacted some people locally but i like to throw things out on the internet sometimes. NYC's market is saturated and sometimes on the internet I can find someone good with some time to put into it immediately. I contacted one of the people on the other post, we will see what comes in.
