Go to Page Layout>Page Borders>Apply To. Select the 'first page' option.

My sincerest thanks to one and all for solving this problem. I now have a complete two-page form which will satisfy my web administrator's requirements for a single attachment to an email posting. I also learned that, even though I know Word fairly well, it should not be used for layouts because of its inherent limitations.

I wouldn't go that far. Rather, you need to understand how Word works (and its limitations) to make the most of what it offers. Many professional-looking publications have been produced with Word.
