Oddly, outline has stopped behaving.

When I click on any outline format including bullets, the first level indicator appears, I type something, hit return and the next line appears at the same top level as the first.

However, when I tab at the second line, instead of the tab dragging the level indicator to the right and changing to the next lower value the tab simply moves to the right leaving the top level indicator unchanged and stationary?
