Default sequencial numbering keeps breaking

I am using a simple outline list for a training document. At various times the sequencing starts all over again at 1. It seems that I should be to correct the formatting and all the subsequent numbering would adjust.



For example, using the copy format feature I copy the format of an earlier line at the same level. This changes the line that was 1.1 to 4.1 (for example). Then I try to use continue numbering on the next line but nothing happens. In this example the second line keeps the 1.2 numbering.

Why don't the two sequential lines connect with the same numbering?

I end up having to reformat all the other lines for the rest of the document.

This is very frustrating. I have had to reformat the rest of the document whenever this break occurs. The document is quite large so this becoming a serious issue.
