Default check box will not activate

I have entered a checkbox. I have tried to protect the document by ignoring #1, using fill in form for #2, and #3 is grayed out and cannot seem to activate form.

Which kind of check box did you choose? Make sure that you didn't pick one of the ActiveX controls found under the "ActiveX Controls" label at Developer tab | Legacy Tools.

If you did choose a forms check box, make sure that the document isn't in Design mode. Click the button on the Developer tab to turn off the setting.
