How do I insert Tabbable Fields?

Hello all,

This is my first post. I was given a document with fields I could type in and tab through. I was looking to create a word document like this but I do not know how to add these fields in. Any input would be much appreciated and thank you very much!

Ive attached a document to show what was given to me.. It's the gray areas that you can insert information and tab through

Your document has been created with Word's formfields. You can access the tools for creating them via Developer|Controls>Legacy Tools.
