Default Missing images in a document

Using Word 2008 for Mac.

My book manuscript includes a cover image on the first page, a different image that I've used at each chapter start, and another image for each scene break.

I've successfully used this document to create both MOBI and EPUB files, and the images have survived conversion without a single flaw.

Then today when reviewing the files, I happened to open the source Word document and was surprised to find that every image except the cover has disappeared.

If I right click on the space where the images used to appear, I get the bounding box with the "handles," but there's nothing in it.

Has anyone ever heard of this? Know how to fix it?
