Creating a special Document

I have been tasked with creating an information report for my company. The format that I want ot use is similar to a Police Intel Report.

I want to basically create a number of boxes within in a word document, with drop down menus etc for sex, hair colour,style etc and areas on the page to allow free text but only within a certain area, but do not have a clue where to start.

Any help would be appreciated.
