Default Form with text in multiple line in a fixed space

Hello,

I have a medical form a physician has to fill after a physical. It consists of some check boxes and mostly of text to be entered with the doctor's notes. He should be able to write multiple-row texts, but only where allowed and within the space available (it all has to stay within the page).

I created a table and it has to stay within the page (landscape). I also put dotted lines where the doctor should enter the text, but the problem is that when he hits the return stroke, the the lines below are moved down, and the table size exceeds the page.
I tried unchecking the autofit option in the table properties and setting a fixed (exact) height for the cells, but it doesn't work.

I understood that I have to protect the file and allow editing forms only.

So I am trying to find a form for multiple-row texts.
  • If I use the text form, then it's just a short grey segment, it does not highlights the whole space available (represented by the dotted lines)
  • If I use the text box (under Active X Controls), the user cannot hit the carriage return and start a new line within the box.

So in both ways I cannot do what I want. If you want to have a look at the file, I uploaded it here

Thanks

Hi Isadora,

Your link doesn't have a copy of the file - only a picture - so it is hard to diagnose.

You can attach a copy here, via the paperclip symbol on the Advanced screen.

Hello, you can actually download it from the link provided. either clicking CTRL+S or chhosing File > Download.

Anyway, I am attaching it here directly (I didn't notice this function before).
Thank you!

Hi Isadora,

See attached. Instead of one table spanning the whole page, I've used a two-column page layout, with a table in each column. As part of the re-formatting process, I was able to delete many unused (merged) rows on each side.

Good day, Paul. This is great. We're trying to do something very similiar, but using Word 2003. Is there anyway to accomplish the same document in Word 2003?

Hi,

The form is already compatible with Word 2003, as it uses formfields. Simply save it in the doc format and it'll be in Word 2003's native format as well. I've updated the attachment to include formfields in the table cells that previously lacked them and with forms protection, so it can be used as is. Of course, you'll want to make some changes to suit your own needs, which mean unprotecting the form, but that's no big deal as there's no password.
