The form is already compatible with Word 2003, as it uses formfields. Simply save it in the doc format and it'll be in Word 2003's native format as well. I've updated the attachment to include formfields in the table cells that previously lacked them and with forms protection, so it can be used as is. Of course, you'll want to make some changes to suit your own needs, which mean unprotecting the form, but that's no big deal as there's no password.
