Question Word 2010 - DOCX File with fields -> PDF with fields

I've attached the file that demonstrates my problem. It contains some fields (which are available by Alt-F9), and I want to save this file as a PDF. Also this PDF should contain all these fields. How can I have this done? Word's "Save As" saves just PDF, w/o any embedded fields.

If you have the Adobe Acrobat add-on with your version of MSWord, simply click on the "Create PDF" tab/icon and save to your location.

For simplicity sake, I created your file to .pdf for you in the event you do not have the add-on available.

Edit: Ooops! Disregard! I was not paying attention to the embedded fields portion of your question. Unfortunately, I don't know the answer to that either. Sorry...

Thank you for your attach, but it contains no AcroFields as well. I use iTextSharp library for getting access and stuffing these fields in the expected PDF, but no luck.
