For the affected section, you must set the placement of endnotes to "End of section" and then select "Suppress endnotes" (in the Page Setup dialog box). To be clear, this must be done for all sections except the one where you actually want to show the notes. For more, see http://word.mvps.org/faqs/formatting...TextAfterNotes.
To insert a TOC and specify the heading levels that you want, select and deselect styles as appropriate in the Table of Contents Options dialog: On the References tab, click Table of Contents | Insert Table of Contents, and then click the Options button.
