Default Toc problem

Please have a look at the attached file. I have created table of contents but there are some problems.

can anyone fix it?

There is a missing tab stop for the TOC entries. Place the insertion point in the first TOC entry. On the Home tab, click the dialog launcher button in the Paragraph group. Click Tabs. Specify the desired location for a left-aligned tab stop. Click OK. Repeat the procedure for the other levels of the TOC.

The reason that the above works is that TOC styles (which control the formatting of TOC entries) are set to update automatically by default.
