Default New boxes to complete depending on input

I am very new to creating forms, but what I want to do is:

Ask a question with a y/n answer. If the answer is y then pop up a box asking for further information.

Hi Jill,

I'm assuming you're using formfields, rather than a userform. For some code ind instructions to get you started, see: http://www.techsupportforum.com/foru...de-606391.html
The code in that link uses a checkbox to trigger the creation of the additional content, including the insertion of additional formfields.
