Unhappy Unable to get a password protected form to perform multiple tasks

I'm trying to make a form that has all the elements that the owner requests, but am having a terrible time. Thanks to some people on this forum, I've gotten the parts to work, but not all together. Could someone, please, tell me if it's even possible? Talk about frustrated.

Requirements:


1. Word document
2. Password protected
3. Multiple tables
4. Table in the middle of the document with a way to add more rows (thank you, Macropod)
5. Data entered in top two rows of first table repeat at the top of all following pages

I'm stuck at 5. I can link the content controls (2 rich text and 5 date pickers) in the first two rows of the form to the header on page 2 (which in theory should continue with the additional pages), but it doesn't seem to work when the form is password protected. Is there a way to get the links to work when the form is password protected?

Or is there a macro that will insert a row (with the content controls) into the table so there doesn't need to be multiple tables? Which should let the top two rows repeat.

(Or maybe I've broken the thing, because Quick Parts (abstract, et al) doesn't work the way people say it should and I can't get StyleRef to work either.)

Rather than fields, I recommend using linked Content Controls to do the repeating text.

If you could post a sample document, we could solve the issues more quickly.
