Default Creating a template with repeat of fields and user input

Good day all

I have created the attached template and have tried using pieces from different solutions provided in the forum. Thank you to all that have contributed.

My intention is to ultimately create this as a UserForm, however I would like to start by crating a normal template first.

The form will basically capture details through fields and drop down lists and assign values to the respective fields.

I have the content repetition working great through the use of CustomXML.



Any guidance and assistance will be great.
