Default userform capture and use vba to enter

I am a beginner at Microsoft word, I am looking for a userform and input the capture text in userform and inserted via vba code (macro) into specific bookmarks on the contract (essay), somebody out there must have done something like this, looking for a sample book and I can work form there, can somone point the finger in the right direction?

There are plenty of examples here that you could check out via a search for 'userform'.

here is a sample form of what I want to do, I have gotten this much from tutorials and guides, but its overwhelming for newbie like me.
Anyhow, I want to use a macro to capture the fileds entered into the user form automaticatly,. I have created a button but not sure how to make it clickable. Please help, thanks in advance.
