Default template text selection based on input multiple choice question

I am creating a template in which certain parts are variable, dependable on input from the user. The input should be asked under the form of a multiple choice question.



So, when starting the macro, a multiple choice question has to pop up, according to the answer(s) that are checked by the user, the corresponding text is shown.

How should i put this in a macro?
