Default Idiot Proof Entry with InputBox

I am trying to find a way to create a macro or something that would simply pop up a box asking for input. This would be simply input like first name, last name, address.



I am trying to get it so I can have First Name, Last Name, Address, and an appointment date entered and then automatically placed to their corresponding places in a word document. Mainly trying to get this done without the use of templates as I do not have admin access to our computer systems and it would just take more work to get it implemented to all who use this form if at all possible.

You would need to create a macro enabled document/template with a userform - see http://www.gmayor.com/Userform.htm for the basics.
