Default Create New Document with responses from a questionnaire.

Hi I have a word doc where there could be 10 or more questions. The questions are answered via a checkbox with the variables "Agree" or "Disagree".



Each Agree or Disagree checkbox has auto text assigned to it.

Can the user select the appropriate check box and when all questions have been answered a button is clicked and a pre designed template is populated with the appropriate auto text.

I have uploaded my scenario, but have no VB code as yet.
